Purpose: Site visit conducted due to ongoing delays and concerns raised by the funding partner regarding quality of work. Assess work complete and outstanding. Confirm schedule and scope of work to be completed
OVERVIEW
All structural work per the drawings and scope of work agreed between Architecture for Humanity and MERLIN is complete. All work per the contract is to be complete by end of next week (March 30, 2012). Additional staff are to be deployed to ensure completion.
ASSESSMENT / OBSERVATIONS / WORK TO EB COMPLETED
Work was assessed room by room. In all cases the outstanding work is described with an estimate of the time required.
Water Tank:
• needs tap and chlorination system to be installed [#2hours]
• installed, and plumbed to gutters, base complete
Urgent Care Room:
• install sink (inlet and outlet plumbed, sink is present) [#1 hour]
• finish painting (around new door) [#30min]
• tidy up [#30min]
• ceiling complete, no evidence of leaks
• wall and window installation complete
• door reinstalled
• bug screen installed
• painting complete
Maternity Room:
• install sink (inlet and outlet plumbed, sink is present) [#1 hour]
• check water supply to shower
• install cabinet [#3 hours]
• tidy up [#30min]
• ceiling complete, no evidence of leaks
• bug screen complete
NOTE: the shower walls are not flush to walls and floor as they are a separate structure not tied into the walls or floor. This is not a structural defect for the building but should never have been built this way
Toilet:
• check water supply to sink and shower
• wall complete
• door reinstalled
• paint complete
• bug screen complete
Staff Room 1:
• need painting finished at B302 [#1 hour]
• wall complete
• door reinstalled
• paint complete
• bug screen complete
NOTE: minor surface crack in wall due to differential settlement
Staff Room 2:
• chase out repair and redo – extg repair is not per the drawings and will chip out over time [#4hours]
• replace/fix bug screen – extg screen is too big and should either be fixed to metal frame or thick enough to sit proud of metal screen. Remove construction waste trapped between louvres and screen [#3hours]
Pump:
• install rubber isolator base and fix to base [#2hour]
• in place and operational
Hospitalisation 2:
• bend vertical steel (exposed from demo) inward to avoid injury [#15min]
• walls complete and painted
• bug screen complete
NOTE: one existing door fell out due to rotten frame. AFH recommend this should be replaced. This will require a Change Order.
Consultation Room 1 / Doctor:
• install new doors [#1 hour]
• paint wall between rooms
• slab complete
• bug screen complete
NOTE: Suggest existing doors do not be reinstalled due to poor condition. New doors should be installed instead. Need to confirm with client. Will require a Change Order
Consultation Room 2:
• install new door [#1 hour]
• bug screen complete
Lab:
• tile counter top – requires prop in corner and replace plywood section in corner [#5hours]
• wall complete and painted
• bug screen complete
Toilet 2:
• Check water supply for sink and shower
• Shave door so it closes more smoothly [#1hour]
Plumbing:
• tank on roof is installed and plumbed
• plumbing is routed to all rooms from the roof tank but needs to be tied into existing facets.
• water flow could not be inspected as work si not complete. All facets and pressure flow rates to be checked before completion
Gutters:
• Gutters need to be replaced and are not acceptable.
NOTE: There is no good way to re-install given the condition of the existing roof structure. Recommend: fix additional 50 x 75mm joists to the existing rafters. Install a new fascia board. Using new brackets fix the gutter in place to falls. [#2-3 days]. Needs to be confirmed with client – may require a Change Order
Roof:
Details of roof rehab are vague – need clarification – the below needs to be checked and approved by Merlin
Option 1: remove approx 8sqm of roofing sheets where roof meets parapet. Replace sheets and flashing. Pour concrete flashing to flat roof to prevent leaks [#1day]
Option 2: As above except instead of pouring flashing to flat roof, cut out flat roof and allow pitched roof to drain into the gutter [#2days]. Requires Change Order
Recommendation 1: the pop up roof on the concrete roof needs to be replaced. It is unsafe as it is not fixed to the roof and could easily blow away. The roof sheets, structure and blockwork stub walls should all be replaced. Requires Change Order [#3days]
Recommendation 2: In the long run the entire roof (sheeting and structure) needs to be replaced. Much of the structure is rotten and the sheets are at risk of leaks. Many already exist. It is not worth simply replacing the sheets given the condition of the structure. Though this is not urgent, it should be considered before too much money is spent fixing the existing roof.
CLARIFICATIONS REQUIRED
• Doors: Replace existing doors to consultation 1 and doctor? And broken door to hospitalization 2?
• Roof Repair: proceed with Option 1 and Recommendation 1?
• Gutters: proceed with recommendation?
NEXT STEPS
• DG to seek clarifications from MERLIN
• Natalie Desrosiers (AFH) to take over site next week and get job complete
• UP and RB to be on site next week and close out
Site report Feb 16 2012
Work on site has stopped and the contractor is awaiting funds to continue work.
- Plumbing work is 0% complete and has yet to be started.
- Insect screen fabrication is 0% complete.
- The gutter has been replaced and is of poor quality. The fixings are not adequate and are poorly attached to the truss. The gutter slope has been checked with water and it has failed to flow. The gutter will need to be re-hung.
- The paint is 25% complete. Only new retrofitted walls have been painted with 1 coat. The entire exterior is awaiting 1 final coat of paint.
- Rubble from construction is still on site and is awaiting the contractor to remove it. This will be carried out when the contractor resumes work.
- Wall 9 has been crepisaged despite instruction to fill it. This wall needs to be blocked off as per the drawings. This will be carried out when the contractor resumes work.

Week 2 has been slow at Turbe
The walls are on schedule to be complete by Monday 6th February. But none of the plumbing work has been carried out.
New 2 cell blocks have been delivered to site. This block has passed the 6ft drop test. All new walls since last week have been completed with this.
The door opening that was built between wall 8 & 9 has still not been ammended despite instruction to fill the wall as per the drawings.
The steel for the floor slab (19) was laid incorrectly. The workers have been instructed on how to carry this out correctly.

The AFH supervisor 'construction outreach' will concentrate on advising the contractor to begin the plumbing work and get work back on schedule.

Site report Jan 25th
Construction has begun on Turbe
75% of the retro fit walls have been demolished and rebuilt. 50% of the retro fit walls have been crepissaged. 0% of the retro fit walls have been endesaged.
The 3 cell block that was on site Wednesday 25th of January is to be removed from site and no construction is to be carried out with it. 2 cell block is to be bought for the retro fit of the walls.
The door opening that was built between wall 8 & 9 is to be ammended to reflect the drawings. There is to be no door opening in wall 8&9.

Observations and Conclusions:
This site consists of 3 separate structures constructed of masonry block, concrete columns, and concrete/masonry and wood/metal roofs. In general, these buildings suffered very little damage directly from the earthquake. Rather, they have experienced varying types of deterioration to: exterior creppisage finishes, wood roof framing members, ceiling surfaces, concrete columns, and concrete floors. The primary sources of this damage is exposure from rain water to wood members and reinforcing steel. Repairs to these buildings will be mostly cosmetic in nature.

Background:
This is a single-story, U-shaped facility that acts as the medical clinic for the Turbe area. The structure has suffered some damage from the January 12, 2010 earthquake, and has experienced various levels of structure deterioration not associated with the earthquake. The building has been constructed in 2-3 phases of concrete block with a wood-framed roof. No budget has been established for repairs.
Observations:
Earthquake damage consists of the following:
1. Lateral displacement of 5 interior masonry block walls in the south wing,
2. Lateral displacement of an exterior wall at the northwest corner of the west wing,
3. Minor cracking between masonry walls and concrete beams and columns,
4. Separation of an attached bathroom from the primary building on the south wing,
5. Minor hairline cracking in creppisage surfaces,
6. Minor separation between perpendicular walls.
Non-earthquake related damage consists of:
1. Deteriorated wood framing,
2. Cracked creppisage wall surfaces,
3. Settled floor slabs,
4. Deteriorated painted wall surfaces,
5. Settled steps and foundations.
Conclusions and Recommendations
Repairs to this building will involve removal and replacement of damaged walls, creppisage repairs at minor cracks, and some cosmetic repairs.
